IMMUNIZATION REQUIREMENTS

A Student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child-care facility or public or private elementary or secondary school in Texas.

                                                                                                                                               

REQUIRED DOSES

 

 

Diphtheria, Tetanus, and Pertussis

(DTaP/DTP/DT/Tdap/Td)

Five Doses of a diphtheria-tetanus-pertussis containing vaccine, one of which must have been received on or after the fourth birthday; however 4 doses meet the requirement if the 4th dose was given after the 4th birthday.

Ages 7 years or older

(pertussis vaccine is not required) Tdap/Td/DTP/DTaP or any combination of these

Three doses including one dose on or after the fourth birthday

Booster Dose

Tdap/Td

One dose is required ten years after last dose of DTP/DTaP/DT

Polio Four doses of polio vaccine one of which must have been received on or after the fourth birthday; however, 3 doses meet the requirements if the 3rd dose was given on or after the 4th birthday.

 

 

Measles, Mumps, and Rubella (MMR) Two doses of a measles-containing vaccine with the first dose on or after the first birthday; second dose required upon entry into kindergarten

 

 

Hepatitis B Three doses

 

 

Varicella One dose on or after 1st birthday

(Two doses if vaccine given at 13 years of age or older)

  PARENTS - PLEASE MAKE SURE YOU SEND A NOTE WITH YOUR CHILD IF YOU WISH FOR HIM/HER TO HAVE MEDICATION AT SCHOOL.  THE MEDICATION MUST BE IN A PROPERLY LABELED CONTAINER - THIS IS FOR ALL PRESCRIPTION AND NON-PRESCRIPTION MEDICATIONS.

Although some sick days are inevitable, there's a lot we both can do to minimize the spread of germs and create healthier environments for kids at school -- and at home.

3 Easy Ways You Can Help:

1. Cough into your elbow, not your hands!

2. Try not to touch your eyes, nose, or mouth.

3. Wash your hands before and after you eat.

4. STAY HOME IF YOU HAVE A FEVER!!
